Why Florida leads the nation in ACA enrollment: Florida's decision not to expand Medicaid under the ACA means hundreds of thousands of low-income Floridians rely on marketplace plans instead of Medicaid. Combined with a large retiree population aged 55–64 (pre-Medicare) and a significant self-employed workforce, Florida has consistently ranked #1 in ACA enrollment since 2016.

Florida's Medicaid Gap — Why So Many Floridians Use ACA Plans

Florida is one of the few states that has not expanded Medicaid under the Affordable Care Act. This creates a "coverage gap" for adults earning between 0–100% of the Federal Poverty Level who don't have dependent children — they don't qualify for Medicaid, but also earn too little to receive ACA premium tax credits (which start at 100% FPL).

This policy decision is one of the primary reasons Florida leads the nation in ACA marketplace enrollment. Adults earning above 100% FPL who would be covered by Medicaid in expansion states instead enroll in ACA marketplace plans in Florida — often with significant premium tax credits.

Key fact: An estimated 800,000+ Floridians fall into the Medicaid coverage gap — they earn too much to qualify for Medicaid but not enough to qualify for ACA subsidies. This is a known policy issue and was a major topic in Florida's 2024 legislative session.
